We offer chronic disease specific care management programs for our clients with a chronic condition such as diabetes, congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD) and many other debilitating conditions. The goal of our chronic care programs is to prevent acute exacerbation leading to hospitalization. We focus on keeping the patient well and medically stable. We also provide in home services for individuals in need of care 24-hours a day, 7 days a week.
